Health and social services Transformation Fund

A £100m Transformation Fund will support development of new models of health and social care, through the work of Regional Partnership Boards (RPBs). It is intended to meet the extra costs of introducing these new models.  

The fund will test the wider adoption and scaling-up of effective new ways of working. These new models could then replace or reconfigure existing services. 

In particular, the fund is designed to: 

  • Assess the ability of a proposed model to expand ('scalability'). This could be from a locality to a region, or from a region to other regions, and 
  • Test whether they are ‘transformative’ (affordable and sustainable). Do they change or replace existing approaches, rather than adding an extra layer on to them.

A Healthier Wales 

In June 2018 we published A Healthier Wales: our plan for health and social care. This was in response to the Parliamentary Review of Health and Social Care in Wales. 

It describes how a Transformation programme will work to: 
  • Implement specific actions set out in the plan, and 
  • Support and encourage a wider commitment from all the main stakeholders and partners   
The Director General, Health & Social Services leads the programme. An internal management board and external Transformation Programme Advisory Board provide support.  

Guidance and next steps 

The attached guidance is for RPBs and health and social care partners. It describes how the Transformation Fund will: 
  • Select and prioritise proposals against the design principles in the plan, and 
  • Be managed flexibly, to allow new models to be introduced and scaled up.
