Data workbooks and explorer tool based on incidents reported by NHS providers in Wales to the National Reporting and Learning System for September 2021.
This is the latest release in the series: Patient safety
This dataset is used to measure levels of reporting from NHS organisations including regularity and timeliness of reporting. It is not suitable to make comparisons between health boards or for changes over time due to the difference between when an incident occurred and when it was reported.
The dataset should be used alongside other local patient safety intelligence and expertise. Organisations are encouraged to report incidents to support the NHS to learn from those and deliver improvements in patient safety.
The monthly publications are management information and not counted as official statistics.
Organisations upload patient safety incidents to NRLS when they can; there is no specific requirement for them to do this on a monthly basis. Hywel Dda LHB, Powys LHB and Velindre NHS Trust did not provide data for August 2021.
Discontinuation notice
This release has been discontinued. NHS organisations are no longer expected to upload incident data to the National Reporting and Learning System (NRLS).
Due to the NRLS being decommissioned at the end of March 2022 by NHS England / Improvement, a decision was made for NHS organisations in Wales to cease upload of data to NRLS for incidents reported on or after 1 September 2021 and to focus on implementation of the new Once for Wales Concerns Management System (OfWCMS).
Future requirements and needs of this patient safety data is currently being explored.
