The reports present the findings from the final stages of the evaluation, focusing on the plan's implementation and the provision of person-centred care.

The final stage of the evaluation involved:

  • interviews and focus groups with people living with dementia and their carers across Wales
  • interviews with practitioners working with people with dementia, their carers, and families
  • a review of Regional Partnership Board (RPB) Dementia Action Plan (DAP) activities for the year  2023 to 2024
  • a synthesis of the findings from the first and second phases of the evaluation, concluding with a final summary and a series of recommendations to guide the successor arrangements for the DAP
