Please follow the instructions for your local health board if you think that you should have had an appointment by now.

Unpaid carer

If you are the sole or primary carer (unpaid) for an elderly or disabled person who is clinically vulnerable, you are now eligible for a vaccine. Please visit the ‘Get a COVID-19 vaccine as an unpaid carer’ page for more details.

COVID-19 vaccinations for household contacts of immunosuppressed adults

If you are aged 16 or over and a household contact of a severely immunosuppressed adult (gov.uk), please complete the self referral form on the NHS website.

Aneurin Bevan University Health Board

Aged 80 or over: contact your GP.

Aged 40 or over or aged 16-64 with certain underlying health conditions: complete the form on the Aneurin Bevan Health Board website.

Betsi Cadwaladr University Health Board

Aged 40 or over or aged 16-64 with certain underlying health conditions complete this online form.

Cardiff and Vale University Health Board

Aged 40 or over or or aged 16-64 with certain underlying health conditions: visit the Cardiff and Vale University Health Board website.

Cwm Taf Morgannwg University Health Board

Aged 40 or over or or aged 16-64 with certain underlying health conditions: visit the Cwm Taf Morgannwg Health Board website.

Hywel Dda University Health Board

Aged 40 or over, or aged 16-64 with certain underlying health conditions complete this online form, call 0300 303 8322 or email COVIDenquiries.hdd@wales.nhs.uk.

Powys Teaching Health Board

Aged 40 and over, adults who are clinically extremely vulnerable, adults with underlying health conditions, unpaid carers and frontline health and care workers complete the Priority Access Form
 
Reserve lists remain in place for people aged 30-39 and people aged 18-29.

Aged 80 or or over or clinically extremely vulnerable: contact your GP.
