Provides information about how to complete the Test, Trace Protect contact tracing e-form.

Thank you for choosing to provide details of your contacts via the Test, Trace, Protect e-form. By providing us with details of your contacts, you are helping to keep Wales safe and break chains of transmission within our communities.

Follow this guidance to complete your contact tracing e-form. You must complete the e-form by 22:00 on the day that you receive the link. The link will stop working after 22:00. The form will take around 10 minutes to complete. If you are unable to complete the e-form by this time, you will receive a phone call from a contract tracer to confirm your details.


The e-form tells us who you have been in close contact with in 10 days prior to and following your positive COVID-19 test or onset of symptoms. We use this information to give the contacts of people who have tested positive advice about self-isolation. This is an important way to help stop the spread of COVID-19.

If you have told your close contacts about your test result, even those who are exempt from self-isolation, it is still important that you let us know so that we can ensure they receive the correct advice. The requirement to self-isolate is based on a number of factors, including age, vaccination status, type of contact (household or close contact) and specific measures for variants of concern. 

If you cannot complete the e-form or you are identified as one of our priority tracing groups a contact tracer will attempt to contact you by phone.

Accessing the e-form

To access the e-form you will have been sent a secure one-time code to you via text message from NHSWALESTTP. This will include a link to complete the e-form.

Occupation or key worker status

It is important you complete this section for us to establish any cluster areas.

If you are a student or school pupil

Please answer the question ‘What is your job title?’ by selecting ‘Student’.

Please answer the question ‘What is your employer’s address?’ by giving the postcode of your place of learning. If you attend more than one place of learning, please provide the address of the place that you attend most frequently.

Households with more than one positive case

Please note that if more than one member of your household has tested positive, we will send you an e-form for each case. If possible it would be helpful if they are all completed.

Places you have visited

Please include any locations that you have visited 48 hours before your first symptom developed or the date you took your test if you have no symptoms. 

Please answer the question ‘Date you visited the location’ with the date of your most recent visit.

Work and educational locations are very important as this is where we may be able to spot clusters and provide support to help prevent the spread of the virus.
